When you reach out to Veterans Care Coordination™, you are often trusting us with personal details about someone you love. This policy explains what information we collect through vcchc.com, how we use it, who we share it with, and the choices you have.
Who we are
This website is run by Veterans Care Coordination™, LLC (VCC), a Missouri limited liability company based in Lake Saint Louis, Missouri. We also use the registered trade names Seniors at Home, Homecare Assist and All About Home Care.
We help Veterans, surviving spouses and their families understand and navigate the VA Pension with Aid & Attendance application process. We also help arrange home care through a network of home care providers. We work with professionals, such as home care agencies, hospitals and social workers, who refer families to us.
Information we collect
Information you give us
We collect the information you choose to give us through the forms on our site:
- Family inquiry form: the Veteran’s or surviving spouse’s name, phone number and email address; who needs care; your name, phone number, email address and relationship to them; answers to screening questions (explained under “Health, financial and other sensitive information” below); and anything else you choose to tell us.
- Professional referral forms: the client’s name, phone number and email address; a family member’s or other primary contact’s name, phone number, email address and relationship to the client; answers to the same kinds of screening questions; the referring professional’s name, phone number, email address, company and location; and any notes. Some referral forms also ask for the client’s address, an alternate contact, whether assisted living or nursing home care is being considered and, for surviving spouses, whether they were married to the Veteran at the time of death and have stayed unmarried since.
- Contact form: your name, address, phone number and email address; who you are seeking information for; that person’s Veteran status; when and how long the Veteran served; and any notes you add.
- Newsletter sign-up: your email address.
- Job applications: if you apply to work with us, your contact details, résumé and anything else you send.
We also receive whatever you choose to share when you email or call us.
Information we receive from others
Often someone else tells us about a Veteran or surviving spouse. A family member may fill in our family inquiry form for a loved one. Professionals, such as home care agencies, hospitals and social workers, refer clients to us through our online referral forms, our downloadable referral form or by phone. They may share the client’s contact details, a family member’s contact details and answers to our screening questions. A member of our team may then call the client or their family.
Information collected automatically
When you visit our site, it automatically receives some technical information, such as:
- your IP address (a number assigned to your internet connection);
- your browser and device type;
- the pages you view and the website that sent you to us; and
- the date and time of your visit.
When you submit a form, our form software may also record details such as your IP address, browser type and the page you were on.
Some of our forms use Google reCAPTCHA to screen out spam. reCAPTCHA collects information about your device and how you interact with the page and sends it to Google. Its use is subject to Google’s Privacy Policy and Terms of Service.
Cookies and similar technologies
Cookies are small files a website stores in your browser. Our site loads tools from the companies below. Each receives basic information such as your IP address and browser details, and some set cookies or use similar technologies:
- Google Analytics uses cookies to help us understand how visitors use our site, such as which pages they view and how they found us.
- HubSpot uses tracking code that sets cookies and can connect your visits to your email address if you have signed up for our newsletter.
- Pardot, a Salesforce marketing tool, has tracking code that can use cookies to recognize returning visitors and the pages they view.
- Ahrefs Web Analytics measures traffic to our site.
- BugHerd is a feedback tool used to report and fix website problems.
- AddToAny provides the buttons for sharing our pages on X (Twitter), Facebook and LinkedIn.
- Google Fonts and Cloudflare’s cdnjs service supply fonts and icons used on some pages.
- Cloudflare helps deliver and protect our site and may set a security cookie.
- YouTube (owned by Google) may set cookies and collect information about your device on pages that include an embedded YouTube video.
Google Analytics, HubSpot, Pardot, Ahrefs, BugHerd and AddToAny run across our site, including on the pages with our inquiry and referral forms and on our thank-you pages. Some of the companies listed above may also collect information about your online activities over time and across different websites and online services, including when you use our site. To limit cookies, see “How to limit cookies and tracking” below.
How we use information
We use the information we collect to:
- Respond to your inquiry or referral by phone or email. Some of our forms also send an automatic confirmation email to an email address entered on the form.
- Understand whether your loved one may be eligible for VA Pension with Aid & Attendance and help your family through the application process. (The VA decides eligibility and benefit amounts.)
- Help arrange home care through our network of home care providers.
- Send our newsletter to people who sign up for it.
- Review your application if you apply to work with us.
- Understand how people use our site so we can improve it.
- Protect our site and forms from spam, abuse and security threats.
- Keep business records, comply with the law, and protect the rights and safety of our clients, our staff and others.
How we share information
We may share information in these situations:
- Our staff and others who help us. Form submissions are emailed to our staff. Copies of some form emails also go to other people and companies who help us provide our services.
- Home care providers. When we help arrange home care, we share information about the person who needs care with home care providers in our network.
- The professional who referred you. If a professional, such as a home care agency, hospital or social worker, referred you or your loved one to us, we may keep them updated on the referral, such as whether we are able to help.
- Service providers. Outside companies help us run our website and business systems. Depending on the service, they may receive information you submit or information collected automatically when you visit. They include:
- WP Engine, which hosts our website, and Cloudflare, which helps deliver and protect it;
- Zapier and Salesforce, which we use to route and manage referral form submissions;
- HubSpot, which holds our newsletter list and provides website analytics;
- Google, Pardot (Salesforce), Ahrefs, BugHerd and AddToAny, which provide the tools described above; and
- the email services we use to send and receive messages, including form notifications.
- With your permission. We may share information when you ask us to or agree that we can.
- For legal reasons. We may share information when we believe the law requires it, for example to respond to a subpoena or other legal process. We may also share it when we believe it is needed to protect the rights, property or safety of VCC, our clients or others.
- Business changes. If VCC merges with another company, is sold, sells part of its business or takes on financing, information may be passed on as part of that deal.
Health, financial and other sensitive information
Our family inquiry and referral forms ask screening questions about the Veteran or surviving spouse:
- their care needs, such as needing help with daily activities, dementia or legal blindness;
- whether they receive income from the VA, and how much;
- whether their assets are under a set limit;
- the Veteran’s discharge status and wartime service; and
- whether they already receive home care.
These answers help us understand whether VA Pension with Aid & Attendance may be an option. You can leave these questions blank and still send the form, unless the form marks a question as required. However, we may need the answers before we can help your family with the application process.
Your answers are stored and shared like the rest of the form. They may appear in the form emails and referral systems described under “How we share information,” and when we help arrange home care, we may share information about the person’s care needs with a home care provider in our network.
A few requests to help protect you and your family:
- Leave out ID numbers, account numbers and medical records. Please do not include Social Security numbers, VA claim or file numbers, bank account numbers or medical records in our website forms.
- Check before sharing about someone else. If you tell us about a parent, spouse or client, please make sure they are comfortable with it or that you are authorized to act for them.
- Do not use our forms for emergencies. If someone needs urgent medical help, call 911.
How to limit cookies and tracking
Our site does not have a cookie settings tool, but you still have options:
- Browser settings. Most browsers let you block or delete cookies. If you do, some parts of our site may not work as intended.
- Google Analytics. You can install the Google Analytics Opt-out Browser Add-on. To learn more, see how Google uses information from sites that use its services.
Do Not Track
Some browsers can send a “Do Not Track” signal. There is no common standard for responding to these signals, and our site does not currently respond to them.
Emails and our newsletter
If you sign up for our newsletter, we use your email address to send it to you, and we keep our newsletter list in HubSpot. You can unsubscribe at any time by using the unsubscribe link in any newsletter email, or by contacting us at info@vcchc.com. Even after you unsubscribe, we may still contact you about a request you have made, such as a question about care.
How to see, correct or delete your information
You can ask us to:
- tell you what personal information we have about you and give you a copy;
- correct information that is wrong or out of date;
- delete personal information we have about you, including details someone else gave us when referring you; or
- stop contacting you by phone or email.
To make a request, email info@vcchc.com, call 855-380-4400 or write to us at the mailing address under “Contact us” below. Please tell us what you would like us to do and, if your request is about a loved one or client, their name and your relationship to them.
We may need to confirm your identity before we act on a request. If you are asking for someone else, such as a parent you care for, we may also ask for proof that you can act for them, such as a power of attorney. In some cases we may need to keep certain information, for example when the law requires it.
Depending on where you live, state privacy laws may give you additional rights over your personal information. If such a law applies to you, we will respond to your request as that law requires.
How long we keep information
Information submitted through our forms is stored in our website’s database, and copies may also be kept in the email accounts and business systems described above. We do not currently have a set schedule for deleting this information, and form entries are not deleted automatically, so they may be kept for many years. We keep information to continue helping your family, to keep business records and to meet legal requirements. How we protect information
We take reasonable steps to protect the personal information we hold. However, no website, internet connection or storage system is completely secure, so we cannot guarantee the security of information you send us.
Children’s privacy
Our website is not directed to children under 13, and we do not knowingly collect personal information from children under 13. If you believe a child has sent us personal information, email info@vcchc.com or call 855-380-4400 so we can take appropriate steps, such as deleting it.
Links to other websites
Our site links to other websites, including our Facebook, LinkedIn, YouTube and X (Twitter) pages and the social networks our share buttons connect to. We do not control those sites, so please review their privacy policies.

Changes to this policy
We may update this policy from time to time. When we do, we will post the updated policy on this page and change the effective date at the top.
